PREPARING YOUR PROPOSAL
A complete proposal to the Fidelity Foundation includes a Proposal Summary Form, a Proposal Narrative and other required documents listed below.
Proposal Summary Form
The Proposal Summary Form provides us with a brief overview of your organization and proposed initiative. All sections of the form should be completed, with additional detail on your organization and proposal being reserved for the Proposal Narrative.
Proposal Narrative
Please prepare a Proposal Narrative that describes how your project meets the Foundation’s Guidelines and Selection Criteria and addresses the following:
- Organization history, objectives and position in the marketplace it serves
- Proposal request and project rationale
- Points of interest specific to your organization's project proposal (See Selection Criteria for description.)
- How the proposed initiative fits into the larger strategic plan of your organization
- Expectations for what the proposed project will enable your organization to achieve
- The anticipated impact of the project on your organization's long-term potential
- Implementation plan, timeline and persons responsible for the project
- Evaluation plans for the project

- Itemized Project Budget
- Project Funding Sources (In detail. See Section VI of Proposal Summary Form.)
- List of Officers and Directors with Their Affiliations (See Proposal Summary Form.)
- Current Operating Budget
- Most Recent Two Years Audited Financial Statements
- Current IRS 501(c)(3) Exemption Letter
Proposal Format
- Submit only one proposal from each organization; multiple proposals will not be accepted.
- Do not bind, staple or package proposals.
- Limit press clippings or background materials to three pages or less.
Mailing Instructions
Mail all materials and completed proposal together. Delivery by e-mail is not accepted.
